Once you have been provided with a NutriPATH request form by your healthcare practitioner, please contact our customer service team on 1300 688 522 to place an order for a collection kit.

You can pay for your test kit over the phone using your Visa or MasterCard credit card.

Please note we cannot provide a test kit without a NutriPATH pathology form filled in by your healthcare practitioner.
